CSF orbitorrhoea with tension pneumocephalus.

A seventy eight year old man sustained penetrating injury to right orbit about 15 years ago. Later he developed right orbital infection leading to phthisis bulbi. Two months before admission he developed CSF leak from the right orbit, tension pneumocephalous and meningitis. A rare case of CSF orbitorrhoea is reported here along with the discussion on mechanisms and management.

Subarachnoid-pleural fistula after resection of a pancoast tumor with hyponatremia.

Resection of superior sulcus neoplasms is associated with a number of complications resulting from the extensive nature of the resection and the necessity to sacrifice certain adjacent structures. One of the complications of resection is the development of subarachnoid-pleural fistula, with the subsequent appearance of air in the cerebrospinal fluid circulation. We report a case in which a subarachnoid-pleural fistula led to persistent pneumocephaly in a patient who exhibited postoperative hyponatremia, confusion, and gait disturbance.

Tension pneumocephalus: acute neurological deterioration associated with pleocytosis of the CSF.

Tension pneumocephalus is an uncommon condition characterized by dramatic neurological deterioration and massive accumulations of intracranial air. Inflammatory changes in the CSF may mimic an infectious process. In this report we describe a case of tension pneumocephalus in which the patient presented with pleocytosis of the CSF.

A case is presented of a 53 year old patient who developed pneumocephalus three days after lumbar anaesthesia. Symptoms and neurosurgical treatment are described. Liquor leakage and lowered intracranial pressure as are discussed as possible mechanisms.
